ELIGIBILITY INFORMATION

Students must be four years old on or before August 31 of the current academic year.

Students must complete the entire application process in order to be considered for Pre-Kindergarten services.

Applications for 2013-2014 will be accepted beginning March 4, 2013.

Applications may be completed at any Pre-Kindergarten site.

Applicants should call or visit the Pre-Kindergarten Site to complete the application with a Pre-Kindergarten Staff Member. Applicants should bring the following information with them in order to complete the application process:

Birth Certificate (certified copy)

Insurance or Medicaid card (if available)

Verification of Duplin County Residency

*Verification of mother's income (if applicable)

*Verification of father's income (if applicable)

Shot Record

Copies will be made of all documents. The originals will be returned to the parent.

*Income Verification may include the following: W-2 or tax return. If a W-2 or tax return is not available, the following will be considered: current check stub, statement from employer on company letterhead, notarized statement of unemployment, DSS statement regarding child support, SSI letter, TANF letter,etc.
