Duplin County Schools requires students to have the following:
- A certified birth certificate.
- Following immunizations: Diphtheria, Tetanus, Whooping Cough vaccine, Polio vaccine, Red Measles (Rubeola), German Measles (Rubella), Mumps, HIB (if applicable), and Hepatitis B, and Varicella. Must bring a record of immunizations that have been administered.
- A health assessment is required by law and has to be completed within 12 months prior to the beginning of school. The health assessment must be documented on the state health assessment form included in the kindergarten registration packets and obtained from the elementary school nurses. The health assessment forms are available at the local health department and at the Goshen Medical Center locations in the county.
- 911 address and proof of residence (i.e. utility bill, phone bill, water bill, rental contract/lease, etc.) are also needed for school records and registration.
